TMD, Snoring and Obstructive Sleep Apnea: Managing the Complex PatientLast year Dr. Allman discussed the signs and symptoms of OSA and the importance of identifying likely sufferers. Medical literature regarding the dangerous effects of OSA continues to bolster the importance of dentistry’s role in identifying, referring and co-managing OSA patients.
This year we’ll define oral appliance therapy protocol using the medical model as our guide. Dr. Allman will discuss sequencing and managing orthopedic relationships, dental pathologies, snoring and OSA. When should we treat both and does it really matter?
While follow-up documentation is essential for therapeutic success, it can be confusing. Documentation strategies via follow-up questionnaires for TMD, snoring, OSA and Complex dental patients will also be presented.

NM Myth-BustersWhile Neuromuscular Dentistry (NMD) has been in existence for over 40 years there are still forces within the dental profession who continue to spread rumors and half-truths. Many of these people are simply uninformed and therefore speak out of ignorance. While most of the experienced NMD practitioners and dental teams have no problem dispelling these myths, the dentists who more recently were introduced to Neuromuscular Dentistry don’t always feel they have developed the tools to defend something they believe in their heart is true and the only way to practice dentistry. Posterior Composites: A Practical, Efficient TechniquePosterior composite resin restorations have become mainstream. These restorations seal teeth, reinforce remaining tooth structure, allow conservative preparations and satisfy patient desires for natural appearance. However, dentists complain that Class II composite restorations are tedious and time consuming to place and aren’t always predictable. The Politics and Science of Neuromuscular Dentistry 1965-2011Thomas Jefferson, and more recently Dr. David Miller, reminded us that “The price of freedom is eternal vigilance”. Dr. Miller evoked this famous quote in a recent J. of Craniomandibular Practice editorial in response to renewed efforts of certain individuals in the dental and academic community to take away your freedom to practice dentistry, particularly neuromuscular dentistry, in the manner that best serves your patients.The science and clinical efficacy of neuromuscular dentistry is well established in hundreds of scientific articles and textbooks. The attempts to denigrate and destroy neuromuscular dentistry are less well known. This presentation will explore three eras that define the politics that have impacted the science and progress of neuromuscular dentistry. The story of neuromuscular dentistry is not unique in the history of science and medicine. Scientific progress threatens the dogmas and financial interests of entrenched establishment dinosaurs unwilling to sacrifice their self interest for the interest and welfare of patients and profession. This presentation will make you aware of the epic battles of the past which have preserved your right to practice neuromuscular dentistry. It is this knowledge that will help you and neuromuscular dentistry to prevail in the latest attempt to by “establishment dinosaurs” to control the Standard of Care for TMD. Predictable Hole-in-One with the first NM bite on your PatientNeuromuscular treatment begins with creating a balance within the stomatognathic system. Such balance is brought about by first having the patient go through a reversible Phase 1 orthotic treatment. The quality of the first bite that is obtained will determine the success and the length of time that it takes to reach this balance. There are many factors that hinder such an endeavor and therefore attention to detail with respect to Posture, Temporomandibular Joint Decompression, staying within the Zone of Comfort, lack of any Human Interference allows the practitioner to obtain the best possible bite at the first significant appointment. Not paying attention to such detail will lead to either a failure to reach such balance or may result in the Phase 1 reversible treatment to be a lot longer than it needs to be. Such length in Phase 1 treatment time can lead to lack of compliance even by the well-meaning patient.

• The importance of the very first bite in the fabrication of the first orthotic. Why a sloppy bite can lead to the failure of treatment or lead to a prolonged treatment time for Phase 1.
• Why Posture matters and how to make postural adjustments that will lead to the best possible bite at that first appointment.

Getting the Most out of Invisalign–Hint “It's not Fire and Forget”This presentation will begin with a discussion of what clear aligner therapy is and is not when viewed as an orthodontic appliance system, followed by a brief history of the technique. Next will be a review of the processes and people involved in producing a set of Invisalign and how that impacts the fabrication and delivery of the product to the practitioner. It will then focus specifically on the factors that affect the smooth flow of treatment and possible problems. A discussion of the communication guidelines for effective treatment planning and ClinChek review follows using multiple examples. Finally the presenter will review the clinical protocols necessary to deliver an exceptional outcome.

• The participant develops an appreciation for the process of designing and producing an Invisalign series.
• The participant appreciates and understands what is needed from him/her in communication with the Align technical staff in treatment plan instructions and ClinCheck review.

He is co-founder of Dental Sleep Digest, a magazine dedicated to clinical DSM practice and The OSA: Online Sleep Academy, a monthly interactive webinar series for dentists serious about advanced patient therapy education. His journey includes 7 Fellowship and Diplomate awards, Senior International Association for Orthodontics Instructor, 5,000 + hours of continuing education, 25 years of dental practice and the loss of his mother due to untreated OSA. Dr. Ronald A. Mathiasen, MD. (Otorhinolaryngologist)Dr. Ronald Mathiasen is a highly regarded board-certified Otolaryngologist-Head & Neck Surgeon. A Phi Beta Kappa graduate from UCLA with highest honors, he graduated with distinction from UCSD School of Medicine in 1996. Dr. Mathiasen completed four years of surgical residency training at the Cedars-Sinai Department of Surgery in Los Angeles, CA and an additional four years of training in Otolaryngology-Head & Neck Surgery in San Francisco, CA. He has been board-certified by the American Board of Otolaryngology-Head & Neck Surgery since 2007. Dr. Mathiasen has authored several publications and has given many presentations in the field of Otolaryngology. His articles have appeared in such prestigious journals as Otolaryngology-Head & Neck Surgery, Laryngoscope, Journal of Trauma, American Surgeon, American Journal of Surgery, the Journal of Surgical Research, and the Journal of Craniofacial Surgery. His practice with Sharp Rees-Stealy medical group is focused on General Otolaryngology and Head & Neck Surgery specializing in diseases and surgery of the ears, nose, and throat. Dr. Mathiasen has special interest and expertise in thyroid and parathyroid surgery, endoscopic sinus surgery and nasal surgery, otologic microsurgery, and facial plastic and reconstructive surgery. Mr. Ulf Broda, RDT, CDT
Ulf Broda, RDT, CDT graduated from Vancouver Technical College with a four-year degree in dental technology. As a dental technician for over 30 years, he has taken the full curriculum of courses at LVI Global, as well as other continuing education courses at various teaching institutions. He is currently the Advanced Cosmetic/Neuromuscular Manager for Aurum Ceramic/Classic Dental Laboratories in Calgary, Alberta, Canada. He is a member of the IACA, AACD, and is a founding member of the Western Academy of Cosmetic Dentistry in British Columbia.
